Order Form for WooCommerce version 1.6.1 has just been released. It fixes a white-screen bug with empty order forms that only have checkboxes for sold individually products.

WooCommerce products can be marked “sold individually”, which means that you can only order one of each product at a time. The Order Form plugin allows you to show a checkbox for those products, instead of the usual quantity field. You do that by adding individual=checkbox to your shortcode.

When the shopper submits an order form without selecting anything, they get a warning message that they must select something.

Sadly, with forms that only had individual order products with checkboxes, that was getting a white screen! And of course, nothing in the error log. v1.6.1 fixes that. Thanks for your patience, Phillip, while I hunted that one down!
